• Pre-planning: Advance Care Plans, Representation Agreements (RA 7, RA 9, Section 7 and Section 9) and key documents to carry out your wishes.
   

• Creating an environment at end-of-life that supports your wishes and values.
   

• Identifying needs for support outside of our scope of practice, and providing referrals to practical and therapeutic resources.
   

• Providing emotional, practical and spiritual support to you and your people before, during and after death.
   

• Advocacy for your end-of-life wishes.
   

• Facilitating the strengthening of relationships, and assisting with resolving unfinished business in relationships.
   

• Assisting with identifying and/or establishing legacies.
